- Completed and signed visa application form
In English or Croatian.
- Valid passport
Issued within the last 10 years; at least two empty pages; valid at least three months beyond requested visa expiry.
- Recent colour photograph
35x45 mm; not older than 3 months.
- Travel medical insurance
Minimum coverage equivalent to EUR 30,000 for repatriation and urgent medical assistance.
- Return travel ticket or confirmed booking
- Proof of tourism purpose and accommodation
Hotel or other accommodation booking, or day-to-day itinerary with ensured accommodation.
- Proof of financial means
Three months bank statement and three years income tax returns.
- Proof of current social and professional status
- Covering letter
